Staff Hygiene Check Form

Complete it at the start of every shift before service begins. Periodic documents such as hygiene certificates are kept separately. This sheet records visible hygiene on that day and does not replace a health report.

For a late-arriving employee, complete the row as soon as they join the shift. No one should begin service before being checked. Self-checking does not count as a control. The chef or shift supervisor performs the check. A person with a no response does not begin service until the issue is resolved.

Add names to the list on event days or when extra staff are working. If a person who is not on the list begins service, the record remains incomplete. Interns and temporary staff undergo the same check. The hygiene certificate belongs on the employment document checklist, while that day's hair covering belongs on this form. Do not skip one because the other is complete. They answer different questions.

How to use this form

  1. Complete the shift and date fields before service begins. Without a date, the form becomes a list whose day cannot be identified.
  2. The chef or shift supervisor checks each team member in turn. Staff do not mark their own boxes.
  3. Mark yes or no for the hair covering, apron, hands, and visible wounds. A person with a no response does not begin service until the issue is resolved.
  4. Record any wound, sign of illness, or missing apron in the notes. Do not record a diagnosis; record what you observed.
  5. Do not leave a late arrival's row blank. Check the employee when they arrive and note the time.
  6. Sign the form and keep it for at least one week. Repeated no responses show that orientation has not resolved that item.

How this maps to Ritapos

A user account is not a hygiene certificate. Ritapos records who signs in to the system; it does not record hair coverings, aprons, or handwashing. Track these separately. A person with an active account does not begin service if this form shows no.

The hygiene certificate on the employment document checklist is a periodic record. This form is a daily visible check. Keeping them in the same file shows both the paperwork and the person's condition that day. Do not leave a new employee unsupervised until the hygiene item is complete in their orientation record.

This form is a checklist for training purposes. It does not change your municipal, food registration, or occupational health obligations. Confirm local requirements in writing.
