Incident and Accident Report Form
Complete it immediately after the incident, before the shift disperses. A record written the next day loses the exact time and witnesses. Respond first, then complete the form. Do not start paperwork until the area is safe.
This form is not an official workplace accident notification. It is for internal records and training and does not replace notifications to SGK or occupational safety authorities. Confirm local obligations in writing. Record even a small cut. It carries an infection risk and may signal a recurring accident. A slip or argument seen by a guest may also be recorded with a short note.
If the same floor or knife appears a second time, write the date of the first form on the new one. One record may be a coincidence; two records show a pattern. If a guest and an employee were involved in the same incident, complete two forms. Combining them can confuse the witness and location. Verifying the time in the transaction logs provides a clear reference if the timing is disputed.
How to use this form
- Respond first. If an injured person, hot oil, or an electrical hazard still requires attention, leave the form until the immediate risk is controlled.
- Write what happened, who was involved, the time, and the witness in plain language. Record what you observed, not your interpretation.
- Record the location: kitchen, dining area, storage, stairs, or street. Without a location, the action needed to prevent a repeat remains unclear.
- Mark the action taken: first aid, referral to a healthcare facility, equipment shutdown, or calling the manager.
- If there is a related order or cancellation, verify the time in the Ritapos transaction logs and add it to the notes.
- Obtain signatures and keep the paper in the manager's file. If the same equipment or floor is involved again, link it to an equipment fault form as well.
How this maps to Ritapos
The incident record is kept on paper. If an order or cancellation is involved, verify the time in the Ritapos transaction logs. The software does not record the accident itself. If times conflict, the transaction log provides a clearer timestamp than a witness statement.
An equipment-related incident must also remain open on the equipment fault form. Recording it only here without taking the equipment out of service leaves the same risk for the next day. Official notification is separate. This paper may support that notification, but it does not replace it.
This form is a checklist for training purposes. It does not change your municipal, food registration, or occupational health obligations. Confirm local requirements in writing.
