Reservation Record Form
Complete it when the reservation is taken and again on the morning of service. A list kept only on a telephone disappears when the host changes. Record the time, name, party size, and telephone number immediately. Saying that a place was reserved does not hold a table.
If you use an online system, this form is a backup for telephone reservations and changes made at the door. Enter the summary in the Ritapos reservation list afterward. Do not leave the two records in conflict. Do not treat a no-show as a complaint. Mark arrived or no-show in the evening. Set your calling policy according to your own rules; the form does not require a call.
Keep lunch and dinner reservations on the same list, but always record the service time. A name without a time reserves a table for the wrong service. Do not add walk-in guests to this form; it records advance reservations. Put any extra guest added at the door in the notes so the evening count distinguishes reservations from arrivals. If the same name has three no-shows, mention that on the next call.
How to use this form
- During the call, record the time, name, party size, and telephone number. Without a telephone number, you cannot reach the guest about a cancellation or delay.
- Share allergen, high chair, and wheelchair notes with the kitchen and dining team on the same day.
- The host and cashier should read the morning list together. A table known to only one of them can cause a dispute at the door that evening.
- Enter the same record in the Ritapos reservation list. If the paper and screen show different counts, note which one is current.
- Mark arrived or no-show in the evening. Do not transfer a no-show to the complaint form.
- Total no-shows in the weekly notes. Repeated empty tables affect capacity planning.

The Ritapos reservation list is the digital record. This paper serves as a backup during busy telephone and door service. Do not leave the two records in conflict. Record any change in party size at the door on both the paper and the list.

An allergen recorded only on paper is a risk the kitchen cannot see. A customer profile makes notes faster to retrieve when a guest returns.
