Equipment Fault and Maintenance Form
Complete it as soon as a fault is noticed. Waiting until evening to mention it may stop service the next day. You can use the same form for scheduled maintenance. Do not leave a printer that is not working for someone to check later.
If the same device jams for a third time, record it. A one-time restart can be noted in the notes field. Repeated records support a shorter maintenance interval or replacement. Link the service invoice to the closing notes. An unresolved fault is carried to the weekly manager checklist.
If backup equipment is in use, do not close the form for the main device. The backup is temporary. Complete the closing notes when the main device returns from service. Do not leave a printer IP change to a server; record the changed setting on this form. If the next shift looks for the old setting, tickets will fail again. A closure without an invoice number will not appear in the expense report.
How to use this form
- Record the device, symptom, and time. Without a symptom, the technician does not know what to inspect.
- Mark separately if the fault stopped service. Record the temporary solution, such as a backup printer, another refrigerator, or a paper order.
- Record a refrigerator fault on the temperature form as well. The unable to measure entry and this form should show the same date.
- When technical service arrives, record the work completed, replacement part, and invoice number before closing the form.
- Review previous records for the same device. A third recurrence is the point to discuss replacement.
- Sign the form and enter the expense in Ritapos. An invoice recorded only on paper hides the cost.
